These notes are intended to assist you in completing the application form, please read them carefully ensuring that all sections are completed as fully as possible. 

Please note – routes 1, 2 and 3 will close on 1 September 2007. All applications on these routes must be received by the Society ahead of 1 September 2007.

This application form and guidance is for candidates who wish to apply for Graduate Membership of the Society, Registration as a Chartered Psychologist and Full Membership of the Division of Health Psychology. If your application is approved by the Society then you will be eligible to use the specialist title Chartered Health Psychologist.

These notes are intended to assist you in completing the application form, please read them carefully ensuring that all sections are completed as fully as possible. 

Applications for Full membership via the Pre-Qualification Route (postgraduate training in health psychology began prior to 1 Sept 2001) will need to be assessed by the DHP representatives on the Admissions Committee.  It is helpful to describe your employment in terms of full-time health psychology employment equivalence. For instance if over a period of a year, a full time research post only involved health psychology 50% of the time would equate to around six months full time work in health psychology. The DHP need to be sure that you have completed sufficient full time equivalent supervised practice in health psychology. It may be helpful to attach a copy of your CV and list of publications to the application.

The criteria for Full Membership of the Division, and therefore eligibility to be Registered as a Chartered Health Psychologist, changed from 1 September 2001 with the introduction of the Stage 2 Qualification in Health Psychology. The criteria are as follows:

1. For psychologists who commenced their training in health psychology before September 2001, but have not undertaken an accredited training course in health psychology, the criteria will be as follows:

Candidates with the Graduate Basis for Registration, who have not undertaken an accredited training course in health psychology but who have nonetheless undertaken postgraduate training and experience involving research, consultancy and teaching in health psychology, and who are judged to have reached a standard sufficient for the professional practice of health psychology without supervision, may be deemed eligible for Registration. Such training and experience must have lasted for at least five years and must have been gained under the supervision of a Chartered Psychologist. Candidates must demonstrate at the time of their application that their period of training and experience began before September 2001.

2. For psychologists who have completed a training course in health psychology between 1997 and 30 September 2001, the criteria will be as follows:

Candidates with the Graduate Basis for Registration who successfully complete a postgraduate qualification in health psychology between 1997 and 30 September 2001 may apply on an individual basis to have this training course approved by the Committee of the Division of Health Psychology. Where the Committee agrees to approve this course, and the candidate has undertaken a period of supervised experience, normally under the supervision of a Chartered Health Psychologist, developing research, consultancy and teaching skills in health psychology to an appropriate level, the candidate will be eligible for Registration. The total period of training for the qualification and experience will be a minimum of three years. This special arrangement is a short-term measure to allow time for postgraduate training courses in health psychology to undergo the Society’s accreditation procedures. Candidates who complete their postgraduate training course after 30 September 2001 will not be able to take advantage of this arrangement.

3. For psychologists who have commenced but not completed a training course by 30 September 2001, the criteria will be as follows:

Candidates with the Graduate Basis for Registration, who have begun but not completed a Society accredited training course on 30 September 2001, will be required to successfully complete a postgraduate qualification in health psychology accredited by the Society and undertake a period of supervised experience, normally under the supervision of a Chartered Health Psychologist, developing research, consultancy and teaching skills in health psychology to an appropriate level in order to become eligible for Registration. The total period of training for the qualification and experience will be a minimum of three years.

4. For psychologists who commence their postgraduate training in health psychology on or after 1 September 2001, the criteria will be as follows:

Candidates with the Graduate Basis for Registration will be required to undertake either a postgraduate qualification in health psychology accredited by the Society or the Society’s Stage 1 Qualification in Health Psychology. Upon successful completion of the training course or the Stage 1 Qualification, candidates will be required to undertake the Society’s Stage 2 Qualification in Health Psychology or a course accredited by the Society as providing exemption from this qualification.

**IMPORTANT**

Upon election, you will be expected to comply with the Society’s

Royal Charter Statutes and Rules

Code of Conduct and

Professional Practice Guidelines for your chosen Division

(hard copies are available from the Society’s Leicester office).

QUALIFICATIONS

Details of the qualification(s) which make you eligible for Graduate Membership with the Graduate Basis for Registration; if this is a Society accredited conversion qualification or the Society’s Qualifying Examination please also include details of your first degree.

 

 If you are already a Graduate Member of the Society with the GBR then there is no need to submit again confirmation of this qualification.

 

Details of any postgraduate training courses in health psychology which you have completed or are undertaking.

 

 

 

Your application cannot proceed until we have proof of your qualifications; as proof of qualification we will accept:

 

An authenticated copy of degree, diploma certificates and transcripts (transcripts should be provided if you hold a modular degree that required you to follow a specific pathway for BPS accreditation).  Authentication should take the form of an original stamp from the university concerned, AND original signature of an appropriate member of university staff. 

We advise you to send authenticated copies if possible

 

Alternatively, you can send:

Original degree, diploma certificates and transcripts of study; OR

 

Original letter(s) on university headed paper bearing the original signature of the Course Leader, Head of Department, or other appropriate staff confirming the award, or your study, of a qualification.

 

You are advised to send valuable documentation by Recorded Delivery.  Original degree, diploma or other qualification documents will then be returned to you by Recorded Delivery, within 3-4 days of receipt.

 

EMPLOYMENT

Details of your employment since gaining the GBR. It is helpful if you can indicate here the proportion of time which has involved teaching, consultancy or research in health psychology.

 

If applicable, please give brief reasons for any lengthy gaps in your CV (such as time spent on maternity leave or travelling).

 

FURTHER INFORMATION

Here you should give details of who has supervised your work in health psychology and over what periods.

  

If necessary, please include any supplementary information required on an additional sheet in the same format.

 

PAYMENT

The fee for Full Membership of the Division of Health Psychology is £30
